POSITION PURPOSE AND EXAMPLES OF WORK:

To complete assigned contact with youth and community resources for assigned caseload and prepare, review, and maintain the appropriate documentation.

55% Complete contact with assigned youth.


30% Prepare, review, and maintain reports and documentation.
15% Communicate and coordinate with manager and Youth Transition Coach and perform other duties as required.
