1. That Petitioner is a resident of Jackson County, Missouri, now residing at ____________________________________.
2. That until __________________________, 19 ___, Petitioner was the holder of Missouri Driver's License Number __________________________________ duly issued under the laws of the State of Missouri.
3. That on ____________________________, 19 ____, Petitioner received notice from the Director of Revenue of the State of Missouri that Petitioner's driving privilege and the aforesaid Driver's License Number ______________________ were suspended (revoked) for a period of _____________________ beginning ___________________ and ending _______________ for an alleged accumulation of points pursuant to Sections 302.302 and 302.304, RSMo.

WHEREFORE, Petitioner respectfully prays this Court for an order setting aside the suspension (revocation) by the Director of Revenue.

ALTERNATIVE HARDSHIP RELIEF
A Count II may be added praying for hardship relief as set out in Form 451, but this petition for hardship must be accompanied by the exhibits mentioned in Form 451.
